California Children's Services/Medical Therapy Program - services medically and financially eligible children under 21 year of age with chronic, long-term phsycially handicapping conditions, and who go to school in Ventura County or have parents or guardians who live in Ventura County. They provide assistance for families and clients in transitioning their medical care into adult related servides. Includes tools for transitioning to adult life.
