Post Secondary Transition
Planning for life after high school is one of the most important parts of a student's IEP journey. As students begin their high school experience and no later than age 16, transition planning helps students and families set goals for what comes next — whether that's college, vocational training, employment, independent living, or a combination of paths suited to the student's strengths and interests.
This section provides resources to help families understand the transition planning process, including how transition goals are developed, what agencies and services may be available to support students as they move into adulthood, and how to build self-advocacy skills that will serve students well beyond graduation.
Our goal is to help students leave high school with a clear plan and the tools they need to pursue a fulfilling, self-directed future.
